You can use freeware command-line utility mp4box or freeware mp4box GUI to create an .mp4 file from .aac file losslessly (without losing any quality). After that just rename the .mp4 file to .m4a or .m4b (you can use freeware Unreal Commander for renaming files in batches) which can be used in iTunes, on your iPod,...

p.s. Please note that you can extract .AAC files from .FLV audio only files that contain AAC sound (like the ones from BBC) using freeware FLVExtract.
