we have the same issues before. what i did is, i just contacted my supplier and they helped me to reset my password. If your DVR is accessible in the internet, i guess that would be easy for them to resolve your issues. Or probably they could support you via phone or email to provide you with secret password to reset your usename and password.

Btw, what is your DVR?
