I don't really see many Real Media streams anymore. In my previous tests I had lots of problems downloading rtsp:// .rm video lectures. The most reliable download manager for Real Media streams for me was NetXFer. It wasn't ideal. It didn't show any errors, but some files were not downloaded completely, so I was forced to re-download them.
