DVDRip to MP4: How to add DVD chapters to MP4 file?

Although Nero Recode is good at ripping my DVDs to MP4 files, I don't really want to use it, because I had several problems with it before. Besides Nero is too expensive for me.

Is there any free software that can rip DVDs to MP4 files (I prefer to use x264 or some other MPEG-4 AVC video codec) with chapters?
